When is it time to arrange extra care and what does a care pathway involve? This evening provides practical explanations from daily practice.

About this evening
The care pathway of people with dementia can be complex. Care track counselors Jolante Cornelissen and Georgette van Hooff explain how signals are followed up, which steps are possible and what to expect upon admission.

There is room to ask questions and share experiences. This will give you insight into het proces and practical tools for yourself or your loved one.
